Simply put your name and address on the form and present it to your employer, college or school. It is best to use Certified Mail so you have PROOF of Delivery. The forms Require a RESPONSE by your school or employer. If they don't respond your mail receipts are proof that were notified.

The document informs them of the possible consequences they face if they are the ones who forced you to take a HARMFUL injection.

Keep copies and mail receipts of records of delivery. If you deliver by hand you won't have proof of delivery, you do want that proof of delivery. This document may give YOU the ADVANTAGE in this situation.

Without proof of delivery they can claim they never received it. Your HEALTH is YOUR Choice. No one can force you to take this "Vaccine."
