This method might take a little explaining.
1. Standing in the correct spot on the mountain path changes the AI behaviour of one of the guards inside. If you don't stand in the exact right spot at the right time, he'll be in your way later on. Why is his AI affected by 47 standing on one specific square feet of ground? Don't ask me, ask the developers. I just work here.
2. Shooting the nurse in the leg makes her stumble and fall. Without the fall slowing her down, she'll get infected. One shot to distract her is NOT enough, the second shot would only send her running down faster. She has to stumble.
3. The NPCs sometimes have terrible peripheral vision. Sometimes not. This time's sometimes.
4. Shooting the ground through the glass and dropping the gun makes the guy inside open the door to check the distraction, see the gun and go report it immediately.
5. The strategy above doesn't work for the second guy, there's not enough time as he tarries with the reporting (maybe because it's the same gun already being reported). Only ONCE was I fast enough for it to work and I think it was because of glitchy AI.

There. It's really all very simple. A child could do it.
