I would not only ask that they "advise" you, I would DEMAND they prove to you all the things that he says in his points and more. Everything is a "contract in commerce". We are run by commercial law (the UCC).
Always reserve all rights under UCC 1-207 and 1-308. Sign the document as follows:

Why? You (the living man) are representing the all capital letter name (you a corporation). All things in commerce are dead entities. Corpse Corporations. You are a dead corporation in the fictitious commercial courts. You are not there under common law but you don't have to have common law, you can use their commercial law (UCC) against them in the law of contracts. A/R = Authorized Representative.
Look at the signature line on a check with a magnifying glass, you will see not dots (the dotted line) but tiny little letters over and over. A/R A/R A/R. You are signing as the Authorized Representative of the corporation. Look at your name on the check (all capital letters). Sounds loony but it is not.
You know where the banks name is located on the check? In the upper right hand corner. That is where your name is right? You are considered a bank in their commercial law. They are robbing the bank (you).
Add that they agree to pay you damages if they are wrong (this is a big deterrence for them). Or even a lump sum amount if you take the shot period. You are amending the contract and it is yours to amend once you "conditionally accept" it.
You are being asked to enter into a contract under contract law. If you refuse under contract law you lose every time. You through your use of the monetary system (how can you not) having a drivers license and birth certificate etc gives them the right to contract (or coerce or force) with you.
If you conditionally accept their offer of a covid test or a vaccination it is up to them to prove (proof of claim) what you are asking for is provable or incorrect. You can make this stronger by doing it in the form of an affidavit (sign it in front of a notary with two witnesses and have it served on them or send it certified mail with return receipt) and demand that they prove "point for point" or item by item all of your demands and that all of your conditions are met.
You could make it a condition that they pay you 1 million dollars to take the shot or whatever. You conditionally accept and bind them into their own modified contract. They can quit their own modified contract by simply stating that they decline your modified contract.
EVERYTHING is a contract and they are absolutely asking you to change your current agreement or enter into a binding agreement.

With "conditional acceptance" you are calling them on their fraud using their laws. You are playing their game and they hate that. Lawyers and judges count on the fact that you will quit/not respond or argue. If you argue you lose. By accepting the contract you have "bound" them in an agreement that you have simply asked for some clarification on some points they left out or you did not "understand".
You must understand you are not in a "constitutional" court but a commercial court. I don''t care if you are in traffic court or you committed murder, you are under commercial law. They may lie or try to tell you that you are not but you are.
Ever had a bank or utility "change the terms"? That is what you are doing. If they don't respond or do not respond correctly (in affidavit form point by point) then they have agreed to YOUR terms. You are basically agreeing to disagree and be done with the matter if it is they who fails to respond.
They cannot terminate your employment or whatever for agreeing, you agreed to their offer or change of terms with some very valid and legal stipulations. If they do not meet them you win.
If they fire you or whatever sue them for damages stated. It's fairly cut and dried. They have reneged on their own contract. I have used this with success and you can also.

Also, who if anyone signed this illegal crap. Find out who is going to be responsible. Ask for any offer to contract be signed (that usually will kill things quickly). None of these low life assholes want to be held personally liable. I would also once I got a name or someone stupid enough to sign off on something I would include in my conditional acceptance that they are agree to be held personally liable for any damages also.
