Le rapport est écrit du point de vue d'une personne en 2030 qui regarde en arrière sur une pandémie qui a balayé le monde entre 2025 et 2028. Le document indique que ses rédacteurs ont identifié les grandes tendances socio-économiques, démographiques, technologiques et environnementales qui, selon eux, sont susceptibles d'émerger au cours de cette période. Les deux tendances qui, d’après eux, sont susceptibles d'influencer les urgences de santé publique sont "les degrés variables d'accès aux technologies de l'information" et l'augmentation de "la fragmentation des populations selon des critères sociaux, politiques, religieux, idéologiques et culturels."
