It is a booster, for now, and ultimately we may transition into calling it a 3 dose series. The point of the 3rd of what might be the early series, or a 3rd early dose, given o month or so after the second dose in immunocompromised individuals. I think it will probably over the next months be able to be somewhat clearer as more data comes out. Who it's probably not even worth trying to vaccinate and who we should go directly to with one of these prophylactic antibody strategies, rather than vaccination. We already know that, for instance, people who have been treated within the past year with the anti-CD20 monoclonals typically used for certain blood cancers and certain rheumatologic conditions, that they will just not make a response. You can vaccinate them 3 times and they still don't respond. We'll probably have a longer list than that." For the immunocompromised, call them the 3rd dose and a booster for everyone else.

That's where we will be looking at pre-exposure prophylaxis with monoclonals and, of course, postexposure and treatment with monoclonals and other, hopefully we'll have some oral antivirals on the horizon, we can only hope. Because we're not going to be able to protect everyone with vaccination. That's where, also, a serologic correlate of protection would be very helpful so you can test your immunocompromised individuals to see if they have mounted an adequate antibody response and if not, they need special treatment."
