Clock Tower (1995, SNES) is a survival horror game developed by Human Entertainment.
This video explains how to get each ending in the game. It goes through core conditions which affect the ending you get and how to combine those conditions to get the ending of your choice.

After making this video I did more testing and have found few minor imprecisions (mostly related to visiting Mr. Barrows or Secret Room). However, these imprecisions should not really affect you when getting the particular ending you want, as long as you follow instructions carefully. That said, I might come back to the game in the future, do even more testing and update the guide.
Note on how this guide was made: I have done testing of all conditions myself by running through the game multiple times while doing things differently each time (with the help of save/load function in emulator) while keeping record of each run in the spreadsheet and then examining which conditions resulted in specific ending.
In total I have tested more than 40 different combinations across something like 7 "new game" instances. I used existing online guides as a reference to guide me through more "productive" routes and not waste my time on those actions which had less chance in affecting the ending.

05:20 - Check if the box contains the key to the West Wing, as we need to go there. The key is not in the box, so we'll find it in the the bird nest later.
05:50 - Get the perfume, we'll need it to get pass the guard dog in the basement.
06:25 - Witness the death of Ann and get rid of Bobby, so we can progress further.
08:22 - Get the insecticide, as we will need it to get the Gold Key.
08:25 - I decided not to pick the black robe in this room, but rather get it in the Phone Room later.
08:55 - Get the West Wing key from the bird nest.
10:25 - Get the ham, we will need it to get out of the prison cell.
10:35 - Get the Gold Key, so we can go into the Phone Room and get poisoned by Mary.
11:55 - Try to save some time by drinking beverage in hope that the game rolls us poisonous drink, not the energy restore. Didn't work out, so we proceed with going to the Phone Room.
13:05 - Enter the Phone Room. Mary isn't here, so we just grab some useful stuff and then try entering room again.
13:15 - Get the black robe, we'll need it to get past the guard dog in the basement.
13:25 - Witness the death of second friend. This is one of (alternative) conditions to get ending B.
13:50 - Get the Silver Key, we'll need it to enter the Religious Room.
14:33 - Finally, Mary is here so we get poisoned and learn about her evil intentions.
15:35 - Give the ham to the prisoner or you'll die.
17:00 - Witness Lotte's death.
17:15 - Use the piece of wood to knock out Mary and escape. Don't hide in the box as this will get you killed.
18:18 - Because the middle shelf is interactable, we conclude that the game has rolled us the Staff (Scepter) instead of the Demon Idol.
18:30 - Because the game rolled us the Staff, we need to examine the mural, then move the shelf and examine the mural again.
18:50 - Because Lotte is already dead, we need to learn Clock Tower's secret from these books.
21:37 - Get the Ceremony Room key.
23:20 - At this point it makes no difference any more (because both Ann and Laura died), yet we still avoid releasing the crow as a condition for ending B.
23:40 - Get the Staff. We will need it to enter the basement.
24:55 - Use the Staff to enter the basement.
25:52 - Use black robe and perfume to pass the guard dog.
28:38 - Choose elevator floor 3.

18:45 - Go back to the Phone Room to get the Silver Key. If you run into Mary again just leave the room as soon as you can and enter it again until she's gone.
18:55 - Witnessing the death of another friend is unnecessary, but I've decided to show it.
19:15 - Get the Silver Key, we'll need it to enter the Religious Room.
20:35 - Because the middle shelf is interactable, we conclude that the game has rolled us the Staff (Scepter) instead of the Demon Idol.
20:45 - Because the game rolled us the Staff, we need to examine the mural, then move the shelf and examine the mural again.

From what I understand there is no guaranteed way to get this ending. The game should either roll you the Demon Idol or let you into the Phone Room without Mary being present there trying to poison you.

This video demonstrates how to get "alternative" ending D. This does not seem to be a "proper" ending, but rather a game glitch which you can exploit to get ending D in a completely different way. I am not sure if this is emulator specific or whether it works on original cartridge/SNES as well.
Walkthrough explanation. To get this ending you need to:
* start the game using "quick start" (not 100% sure about that, but I was never able to get this glitch when using "normal" start)
* immediately go to the courtyard and run to the left
* if the glitch occurs, you will not witness Ann drowning in the pool
* run to the left to exit the courtyard
* run to the left until you reach the garage
* then escape by car, without witnessing death of any of your friends
00:09 - Start the game using "Quick Start".
00:40 - Immediately go to the courtyard.
01:00 - If the game glitch occurs, Ann will not be present in the pool.
01:30 - Run to the left until you reach the garage.
01:50 - Get the key and use it on the car 3 times to escape.
Rumors around the internet say that it is also possible to get ending F by using the same glitch (Ann not drowning in pool), but I was unable to reproduce it. According to aforementioned rumors, you can enter the lift in the basement without witnessing deaths of any of your friends. However, going to the basement requires getting the perfume from the bedroom first. Entering the bedroom made the "glitch" disappear for me, meaning that Ann will be present either in the pool or falling from the ceiling (tested with emulator save-states). During this playthrough I forgot that the door to Scream Corridor on floor 1 does not require West Wing key, so I could have just ran into it and witnessed Ann's death right away without going to floor 2 and getting the key.
