It's definitely the most memorable:
A literal Jigsaw-style escape room puzzle, a couple 'jumpscares', an explosive trap that's got more and more punishing through the updates, going to grab the Eye Key only to be ambushed by a small army of Spiders (thank FUCK the Mother Spider here has less HP now), but most of all:

This was the first time I found a secret level in DeathWish on my first playthrough. The method for getting in there has changed, what with the 7 Key now being in a completely new secret (albeit literally only a couple of feet away); but every time I see those Console Messages pop up on the path to the Secret Exit, I remember how much I shat bricks on that first discovery.
